We use several different questionnaires and diagnostic tools in an effort to gain a better understanding of your life experience, personality traits and family dynamics. All information is held in strict  confidence.  When  you  contact  us   to  schedule

 your   first  appointment  be  sure  to  ask  for information on counseling rates. You should also note that we do not work with insurance companies regarding payment. If you have scheduled your first appointment please complete the "Personal History Questionnaire" (PHQ) It is very important that the PHQ is completely filed out and brought to your first appointment. We would also like you to download and read the "Client Guide." If you have any questions please feel free to contact the ministry.
